ADOPTION CONTRACT

Rules for Adoption:

1.  The Adopter shall provide the animal with humane care and maintain it in accordance with all current and future state, county, and municipal laws and ordinances where the Adopter resides.  Where there are no applicable laws, the adoptive family shall keep the adopted dog under control either by physical restraint (leash or tether), barrier (physically or electrically fenced yard or enclosure), or in immediate voice control while outside the confines of the home or barrier.  The animal shall never be chained.

2.  The Adopter shall provide the animal with necessary veterinary care upon sickness, disease, or injury.  In addition, the Adopter shall take the animal to the veterinarian once a year for an annual health examination and routine vaccinations (to include DHLP-P and rabies as required), and will maintain the animal on year-round heartworm preventative. 

3.  The Adopter shall provide the animal with an effective and humane means of exercise.  The animal shall never be chained.

4.  The adopter shall provide a high quality diet (for example, Science Diet, Iams, Pro Plan, or Nature's Recipe, etc.).

5. If, for any reason, the animal cannot be kept by the Adopter, the Adopter shall return it to the Collie and Sheltie Rescue of SE VA.  The animal shall not be given away, sold, or exchanged without the prior written permission of the Collie and Sheltie Rescue of SE VA.

6.  The animal shall reside at the Adopter's address.

7.  The Adopter shall keep the animal as a household pet and shall not use it for breeding or exclusively as a guard dog, working dog, or hunting dog.

8.  The Adopter shall provide a collar and identification tag for the animal to wear at all times when allowed to go outdoors.

9.  The adoption donation is non-refundable.

10.  The Adopter, from the date of this contract, agrees to release and indemnify the Collie and Sheltie Rescue of SE VA from any and all claims, known and unknown, now or hereafter arising in connection with the animal. 

11.  The Adopter consents to examination of the animal and the animal's environment ("home check") by the Collie and Sheltie Rescue of SE VA every three months for a period of one year.  After one year, the Collie and Sheltie Rescue of SE VA will remain in contact with the adopter by periodic phone calls.  The Adopter also consents to reclamation of the animal by the Collie and Sheltie Rescue of SE VA at any time for failure to comply with the terms of this contract or for any misrepresentation of fact made by the Adopter.

12.  The Adopter agrees to pay the Collie and Sheltie Rescue of SE VA any and all expenses, including court costs and reasonable attorneys fees, in enforcing the terms and provisions of this contract.

13.  The Adopter and Collie and Sheltie Rescue of SE VA agree that the actual value of this animal is $200.

14.  The Adopter or anyone else residing in the adopter's home has never been convicted of animal cruelty, neglect, or abandonment of an animal.
